D2/D4 Medicals (LGVs, HGVs, Buses, Forklifts & Motorhomes)
This is a medical, designed by the DVLA, to ensure that drivers of LGVs, HGVs, Buses, Forklifts and large motorhomes are medically proven fit to do so. The DVLA D4 form is used for all these medicals. It can be downloaded below.

If you’re looking to drive a Heavy Goods Vehicle (HGV) or a Passenger Carrying Vehicle (PCV) such as a bus or coach, you’ll need to complete a D4 medical as part of your licence application or renewal. Likewise, Forklift drivers and Large Motorhome drivers need to prove to the Driver and Vehicle Licensing Agency (DVLA) that they meet the necessary health and fitness standards to operate large vehicles safely.
The D4 medical assessment involves a review of your general health, eyesight, cardiovascular fitness, neurological function, and mental health. It must be completed by a doctor, GP, or private medical provider. If you are applying for your first HGV or PCV licence, you’ll also need to complete a D2 application form alongside the D4 medical.
Drivers must undergo regular medical renewals:
Every 5 years from age 45 to 65
Annually after age 65
Employers and Transport for London (TfL) also require drivers to be medically fit throughout their careers, so even if your licence is valid, you may need an occupational health check. The D4 form is submitted to the DVLA as part of your licence application or renewal process.
