Motorsport Driver Medicals
To obtain a motorsport licence, drivers must complete a medical as required by governing bodies, for the safety of you & the other racers. These medicals generally use the D4 form, designed for drivers of HGVs and buses. It can be downloaded below.

Competitive motorsport involves extreme speeds and high levels of physical and mental exertion, so medical fitness is a priority. To obtain a motorsport licence, drivers must complete a motorsport medical as required by governing bodies such as Motorsport UK (MSUK) and the Fédération Internationale de l’Automobile (FIA).
Motorsport medicals assess:
Vision and hearing (drivers must meet strict eyesight standards)
Cardiovascular health (ECG tests may be required for older drivers)
Neurological function (checking for any risk of blackouts or seizures)
General fitness (some categories require a physical fitness test)
For Motorsport UK licences, a medical declaration is required annually, and a full doctor’s medical is mandatory for drivers over 45 years old or those competing in high-speed racing categories. FIA international licences have stricter medical requirements, often including ECG and stress tests.
Motorsport governing bodies provide specific medical forms, which must be completed by a GP or motorsport-approved medical examiner. Drivers must also inform officials of any medical conditions that may affect their ability to compete safely.
